Death certificates, 1902-1957.

Record includes Jacksonville patient death certificates filed by physicians with the Department of Public Health. Form entries include deceased patient name, age, sex, race, birthplace, occupation, and marital status; Illinois residency length; death date, time, place, and cause; burial date and place; undertaker name; and physician's signature. Forms (1920-1942) also include deceased's birth date; permanent residence; residency length at death place; United States residency length; spouse name and age; father's name and birthplace; mother's maiden name and birthplace; informant name and address. Forms (1954-1956) also list deceased patient's Social Security number and whether patient served in the armed services.
